Default Clutch Problem?

So its hard to explain so bear with me guys.

On a cold start everything is great.. But say after like 30 min of driving this happens.
I would stop at a stoplight and the put it in first and give some gas and release clutch as ussual, but when everything is warm like it seems it doesn't want to grab right away.. Like it bucks... And its not heat soak, trust me (also some days this wont happen at all, while other days it will happen) ..

also
I can have it in first or second and slowly rising to about 5k then floor it (when its been driving for about 30 min it does this) it will hit vtec and climb up then it seems the clutch is slipping or not grabbing around 8 or so, u can feel the car slow down... (i have also noticed on days that i dont go past 5k it may not show any dificulties until i do pass 5k then it will start again)

Remember this is only when the car has been driving for awhile.. So even if the engine is warm if I wasn't driving it for awhile it wont do this..

I first thought it was the clutch fluid, cause that was extremely dirty, so I flushed it, well its much better but still does it...

So my questions is what you think is wrong, if it was the clutch going out why would it only do it when its warmed up.... Please Post Guys this is getting annoying.

Your clutch is shot. Time for a new one.
It makes complete sense, when its warm its not grabbing.

Hey also to add to it,
today it showed like no signs of anything when I was driving and it was really warmed up I drove it for a long time...
You still think its the clutch...
