Wow thanks guys! Parts that I changed out are:
Part # 12 91005-PCY-003 Honda BEARING, BALL (31X55X13) $16.70
Part # 13 91123-PCY-004 Honda BEARING, TAPER (41X68X20) $32.10
Part # 14 91124-PCY-004 Honda BEARING, TAPER (35X68X19) $32.10
Part # 15 91201-PCY-003 Honda OIL SEAL (40X60X9) $7.20
Part # 16 91301-PCY-003 Honda O-RING (28X2.6) $2.41
This one is the shift rod tube o-ring - 91312-P13-000 Honda O-RING (22.2X3.2) $2.01
As you can see also, the long bolts for the case are part #19 there... which are longer than the short bolts #18. This is what I was referring to in my earlier post about the bolt lengths. Also worth noting, I did have to get transmission fluid and washers, hondabond, and urea grease. I also had to get a 22 dollar harbor freight slide hammer to pop off the outer bearing race from the transmission case. The race is from the old bearing #14. The new one was installed with a wooden block and a hammer. I cannot believe how great the S feels now! It's like a whole new car I hope this is insightful for anyone out there having this issue!

I am regretting not making a youtube DIY of this repair... could have helped everyone out a bit more...
Anyhow, yes the cause for this bearing failure is an interesting question. It is high up in the transmission case so lack of oil would make sense to me. Perhaps there was an improper install from the factory? It will be interesting to see if the problem comes back for those who have had to repair it like I did.
