Bush outer shells
I did wonder if something like this ball joint removal tool set http://www.ebay.co.uk/itm/21PCS-REMO...-/271334165929 would make life easier - but I have no idea if it would actually help or not.

For a poly bushing to work like it suppose it's essential they are well lubricated.
Not fun to remove the arms everytime to get them greased up so fitting grease zerks is a very good idea.
I fitted zerks on my lower front arms and also added some grooves in the bushing so the grease has a chance to stay where it needs to be.

My skunk 2 camber arm bushes have <8>longitudinal groves in them <not S2000> squeaked when using incorrect lube <silicon> would only last a week.Then copper grease used with on prob.
The poly bushes i used<S2000> had impregnated in the molding a mesh type pattern to help retain <on this occasion White Grease>

I'm asking, surely some is? Otherwise wouldn't everyone have problems after so long?
Yeah, I have the same as you, and saw the cross hairs/grooves your referring to, to help keep the grease for the bush collar.
But there isn't any on the external part of the bush which comes into contact with the arm. Which is why these Zerks are being used.
But you would think the ridge/lip on the end of the bush would prevent grease from just coming out?

Yes if you got cross hairs/grooves then the grease should stay where it should at least for a while, but not all the brands got them.
I know my Powerflex ones didn't, I talked to them about that and the informed me they have started to use some kind of knurling on a lot of their newer parts though.
You don't want any movement between the outer part of the polyurethane and the arm, it's the steel collar inside the polyurethane that should move.
If you do some searching there is a lot of people having troubles with bind and squeaking poly bushings, most is because lack of lubrication.
In some cases maybe it will be fine for several years, I don't know I'm just trying to be helpful
