Undo the nut underneath the gearknob (the only hard bit is finding a spanner of the right size, so you don't mangle the nut), unscrew the knob, remove it along with the nut (assuming you're replacing it with a countersunk knob that doesn't require the nut), put the new knob and and screw it up until it is tight enough that it won't undo in your hand at the most inconvenient time mid-change!
