People seem to forget that its just sleeping - you can wake up any time. The dream thieves regulate sleep with sedatives (the suitcase) to keep you under, but if they wear off, you'll wake up. The issue is time in each level of sleep and how each is exponentially faster. In Limbo, so much time passes compared to a normal dream that you'd go mad, and when you woke up, you'd be insane. That was the risk of Limbo in the movie - people seem to forget that and some other important aspects of the movie, but its hard to take so much in. Watch it a second time and its easier to understand - most people who think its a dream change their minds after the second time they watch it.

To get down to Limbo you need strong sedatives. As for waking up on the plane, that is how long the sedatives were timed to last - the big question was would Cobb and Moto wake up sane or insane based on getting out of Limbo.

This is how I saw it and could be wrong.

If you die while in a dream (only if sedated) you go into Limbo. Cobb died in the van, which was the first level of his dreaming, causing him to go to Limbo where Saito is.
There Cobb reminds Saito that they are in Limbo, which Saito believe is his real world. I think it is assumed that they both kill themselves in Limbo, causing them to wake up in the real world (on the plane).
