To get down to Limbo you need strong sedatives. As for waking up on the plane, that is how long the sedatives were timed to last - the big question was would Cobb and Moto wake up sane or insane based on getting out of Limbo.

If you die while in a dream (only if sedated) you go into Limbo. Cobb died in the van, which was the first level of his dreaming, causing him to go to Limbo where Saito is.
There Cobb reminds Saito that they are in Limbo, which Saito believe is his real world. I think it is assumed that they both kill themselves in Limbo, causing them to wake up in the real world (on the plane).
