popping the gears out is a sure sign of rebuild. grinding as well with clutch all the way in.
I've had this, I have a new transmission via honda warranty (thanks rob).
it's expensive. about $5000. but most honda dealers will not rebuild your transmission for you. rather, you probably have to buy a new one.

On a level road the car will accelerate smoothly from 20-35 MPH, in fourth gear, under light throttle, without lugging. Acceleration has to be VERY LEISURELY because anything more than a light application of the throttle will cause lugging.

Here is the TSB for popping out of 2nd. I have not seen 1 for popping out of 4th.
With the AP2 transmissions, the consensus has been that if you have a low milage car that pops out very occasionally when cold the problem will generally resolve itself by 10K miles of so.
If you have a car which is constantly grinding and popping out of gear hot and cold you obviously have a screwed up transmission and need it repaired.

Maybe. Maybe not. It all depends on the throttle setting. Consider the extremes. If you're coasting, the engine can't lug unless the engine speed drops below idle speed (and then the engine is pulling, or trying to pull, so you are no longer coasting, and the engine will bog rather than lug).
