Here is another possibility. If you are trolling in 4th at 25, you are applying very light throttle. There is a point in feathered throttle application where there is neither engine braking nor positive force through the transmission. At this point, the very lightest touch on the shift lever will move it out of gear, whether or not the clutch has been depressed. Perhaps this is what happened. It is generally good practice to avoid leaving one's hand resting on the shift lever while cruising along. Just consider this as a possible explanation.

If your shifting a cold motor at 3K, you get to 4th pretty quick. I hear *some* people shift cold motors at 3000 rpms...

Hahaha, yea, I do shortshift when the engine is stone cold, but I also accelerate like I had all day to get where I'm going. In "normal driving" it's pretty hard to get all the way down to 20 in fourth gear though, 'cause I tend to downshift and use engine braking. Normal driving /= warming up the engine.
