I do not know the physical reason. Here it is from the Rotrex C30-94 setup manual. Google "Rotrex Setup" to see the manual first hand.

" The Rotrex supercharger can only be operated in one rotational direction. Never turn the pulley in the incorrect direction by hand or otherwise. For your particular unit, please find the correct rotational direction in the technical datasheet found on www.rotrex.com"

I literally could not believe this when I read it. Of course, being a really astute individual and researcher, I read it after the Rotrex was installed on my S2000. My Rotrex grenaded after 125,000 miles. I have been NA and happy about it since.

I don't know whats more unbelievable, the 'by hand rotation disclaimer'... or the fact that you managed to get 125k miles on it before it grenaded

So, just so Im on the same page:
if I take off the belt and it makes the same sound, then its a bearing issue?
and if I hand rotate and it’s not smooth, then its a bearing issue?

Also, if it turns out to be a bearing issue, what’s the best/worst case scenario?

If you remove the belt and its still making the same noise, then obliviously its not coming from the blower bearings, right? You can then check elsewhere.

Yes, the blower should spin smoothly by hand, but I guess pay attention to the direction you are rotating it, or it might blow up on you if it isn't blown already

If the bearings are blown, there is no best/worst case scenario. The blower needs to be sent in for rebuild or replaced if it cant be. You can rebuild a Paxton when the bearings fail and its about $500. I don't know about Rotrex.

My blower whines as much as the op and there is nothing wrong with it.

take off the belt, rev motor and see if it still ticks. That will eliminate the blower.
