With my car- Bad Luck.
Sheila's- I don't know. She is an excellent driver so you can rule out "user error" for certain. And takes excellent care of her car, too.
Which brings me to my next question. Bryan and I were discussing this, and we were wondering if pitted valves is a natural wear and tear with high mileage? One of the items on my repair/maintenance list was to go ahead with a leak down test, and should I have leaky valves is that a sign of wear or something more catastrophic?
Just curious really....

Like ruexp67 says, probably from running lean a long time.
We know the O2 sensor was bad, not bad enough to throw a code but slow in tracking the changing fuel mixture. My best guess is that the O2 sensor had been going bad for a long time and was consistently running the car lean.
