sb2007 ophthal. is still nice although in the big cities the market is tight, i'm in ent which is decent and of course i still hear good things about derm and urology.

Nah...not ALL of us sue docs, Steve. Some of us evict little old ladies, lock up the innocent, make shameless deals with the devil, and put babies on spikes. It's all about specialization, brother!

Yeah, I've got two buddies that want to do Optho, and a few who want to do ENT. I've heard from both that if you don't have a 230 on step 1 they will throw your application in the trash.
On match day this year the fourth year at my medical school who was top of his class with >240 on step 1, awesome grades, research experience, had interviews at almost 20 places, and a good personality didn't match in ENT...
I'm not trying to scare you off but there is lots of luck of the draw, lots of people who "know" people, and lots of working your arse off. I don't claim to know what combination of above works, but as you can see by my example you can even have it all and still not match.
