Road Worthiness.........
Here in Atlanta, GA we put allmost twice the national average miles on our cars per year (16 - 20000).
So, most people buy a new one every 4 or 5 years. Where they go from there...??? We also have a laughable emissions inspection program. Espescially when you consider that in 20 years Atlanta has gone from pretty clean air to near the top of the smog list with L.A.
Oh, and to answer the inntial question...no place I have lived or heard of requires any type of "road worthiness" inspection on vehicles.

Massachusetts has annual inspections for safety, lights, tires, etc for all cars.
Last year, if the car was less than 3 years old, this was all that was inspected.
If greater than 3 years, your vehicle is put on rollers (dyno like) with an emission gauge in the exhaust and the car is driven (to 65MPH I think). Must pass minimal clean air requirements.
Last week I went to get my 99si inspected and the owner of the garage said he wasn't sure if it was 2 or 3 years for my car on this. According to him, (sounds messed up, but) different vehicles fall into different categories based on their original emission specs as to whether it's 2 or 3 years.
Summation-Mass is !@#$% up when it comes to this and is still trying to figure it out!
BTW-as with most states here in the US(all????), once you pass a driving test, the licence is basically yours until death Hope to be on the road at 90, but I feel bad for all the other drivers.

In Texas the safety inspection is annual. Only exception is when you buy a new car your sticker is good for two years. Depending on the county you live in you may need a exhaust test. The inspection is a joke when you look at the number of beaters that are on the roads in the Dallas area.
In Virginia, you must have an annual safety inspection for items like tires, lights, windshield, etc. This is required every year, including before the car is even sold off the lot for the first time. The inspection is limited by law to cost no more than $10 which obviously doesn't include the cost of repairs.
As stated previously by mnS2k, Texas has an annual inspection. About 3-4 years ago the state announced that new emission testing would soon be required. Every station had to purchase new, expensive equipment, and many did so. Subsequently, the regs were not enacted and many stations lost their ass. Ain't gov-ment wonderful?
