I would suggest that all these problems are quite probably the result of the jack plug not being inserted properly. In fact, without wanting to alarm you, I wouldn't suggest you power up with the jack plug not correctly inserted. And if you are going to disconnect it you may as well grind it, and reinsert it.

If problems persist, well then we need to get further into it.

Ahh, but have you done anything? Have you tried the plug grind?

oh, one more question-
what does that extra trim do to the unit?
bad connection? how did you guys find that out?

The extra trim prevents the connector from going all the way into the socket. I discovered it by examining the connetor and the socket. There is a metal or plastic shroud on the Sony that is designed to prevent the connector from fitting.

The adapters went out Monday for everyone that either said they needed it, or I did not hear from. Please let me know how they work for you.
