There is an large amount of stress and vibration that applies to that area. It is extremely important to assemble correctly the first time and avoid this if possible. Recheck the torque on those bolts occasionally.
Helicoils help too.
Sideways, have to agree, I had this problem too but we didn't check the bolts enough before. Told my tuner about the way the bolts should be mounted and that they just on the final step should be tightened. Let's see what happens next.

Anyway, I have one part spare at home, just in case this one goes leaking again.
