Default Oil Sandwich Leak

Hey guys,noticed a small leak where my oil sandwich is from my relocation kit. Any ideas what the cause is? From what I know the only reasons would either be:

1.Not tight enough
2.O Ring busted
3.fittings loose
4.installer didnt apply teflon to fittings

any other reasons that might be causing the issue? its a very tiny leak, but overtime since the car is parked for winter its making a bigger puddle...would like to fix it when i install my oil cooler, any input would be appreciated,dont want to always scrub down the floor lol. cheers fellas!

Is it coming from where the plate mates on the block or from the fittings?

If it's from the plate itself, it could be either the oring, or bolt not tight, or sometimes I've seen the plate misaligned and not making full contact with the block surface.

Which sandwich plate is it? I believe the Canton one required the bolt to me trimmed down because it would bottom out on the Honda oil filter fitting.

I have the the greddy one which came with the turbo kit. havent had time to jack the car up and examine it yet, but will be doing that when i put the oil cooler in. Hence I wanted some opinions on what might be the culprit. such a small part, so many things " could " be wrong lol fml
