What Caused It?

I'm certainly no expert, but that's exactly what I'm suggesting (as a possibility). I believe that the #4 cyl runs a little leaner than the other three, and #2 might run slightly richer. If the car was blowing fireballs and smoke from unburned fuel then I have a hard time believing that it was running lean, and I believe that these precursors DO point to a rich condition that might wash the lubrication off the cylinder wall in #2. A plugged or restricted oil jet or banjo bolt could also be a contributing factor.

Of course it could be totally unrelated to the rich running or the oil jets; I'm just guessing. The damage is right between the intake valves, in an area that is probably getting a good shot of fuel, which comes out of the injector and is spread out by the valve head. I think the damage is right in the area where there is a shot of fuel hitting the cylinder wall, and if there was a lot of raw fuel there it would certainly wash out the lubrication, as well as making the car smoke and blow fireballs. Maybe I'm way off base here, but there sure is a lot pointing to both lubrication failure and an overly rich condition.

No flames please... I have no idea, just applying knowledge I acquired here on s2ki.
Is it possible that the head was lifting causing the piston to wobble and score the cylinder? Or would that be a uniform scoring across all cylinders?

Usually I would side on the fuel washing if there was visible signs of the car running excessively rich. However, you should have seen consistency throughout other cylinders. Which then leads to other suggestions such as excessive slap or improper oiling from the oil jet.
