"Foot work" Where is left leg?
I just drove to a neighboring city with stop-and-go traffic in both cities. I have my heel on the floor UNTIL I need to shift. Then, in a fluid motion, I make a transition to heel off of the ground clutching action. I can't only use my ankle to clutch, the whole leg's got to get some.
For me, it depends on the circumstances. Sitting at a red light, my heal is on the floor. Taking off after it turns green, especially in traffic when the start is very easy, my heal stays on the floor. Just about any other time and my foot is hovering over the peddle. It's just what works for me! 
Other than that, my foot is squarely "off" the peddle and "on" the floor. Doesn't matter how brief the interval is going to be before the next shift, my foot is "off" the peddle and "on" the floor at any time that's "not" shifting a gear.

I plant my heel also for normal driving situations, using my ankle to depress and release the pedal. Left foot seems more relaxed since I'm just flexing my ankle, as opposed to my entire leg. Also seem to have better perspective of clutch travel.
Hover works well when I'm shifting quicker.
I also agree that if you can't depress the clutch fully with your ankle, then the seat is too far back...

my lefty is on the dead pedal when not in use while the car is in motion. at a stoplight or something...i rest on the floor. light turns green and i hover on top of the clutch and move it to the dead pedal after completing the shift.
I actually paid attention to what my foot was doing when I clutch - it starts with my heel posted on the floor, and as I push in, my foot comes completely off the floor and I just press, then it reverses when i clutch out. Same thing with moving my toes over to the dp though.
