it's probably because you have air in the system.

believe it or not you can't get all the bubble out in one singel flush, i always bleed one more time after the first week, sometime you'll be surprise how sneaky those bubble can be

Also my rear brake pad appears to have a chunk that crumbled off What could have caused that?
