If the engine is running it's fine. Revving to midway in the band won't hurt the car. I'd not redline the car until she was warmer. These are 55°F/13°C cars, we're not driving them below 0° (certainly not 0°F!).

One possibility for DBW cars is a failing Accelerator Pedal Position Sensor.

I thought I had a mechanical problem with this, but it turns out the heavy engine braking when coming off the gas in 1st was causing my body/foot to come forward a bit, hitting the gas slightly, causing the cycle to repeat. Sounds silly but true, definitely a face-palm moment.
