Why heel-toe?
The S2000 Prototype in Nurburgring video has shots of the drivers feet. Watch and learn. (currently not available though).

Another video with shots of the drivers feet is the Miata vs RX-8 vs S2000 vs WRX vs RSX Type R vs Skyline.

I've been trying to do it a little but I can't seem to get my heel on the throttle very well when braking. As a motorcycle rider I learned long ago how to blip the throttle when down-shifting but doing that in the S while braking is going to take some practice. Guess I'm going to have to put some more miles on it.
Some of us heel-toe'ers don't actually use our heel. Rather, we use our pinky-toe to blip the throttle. However, unless you have very wide feet, the pedals on the S are a little far apart for this technique. Some have bent the accelerator pedal to move it closer to the brake while others have bought aftermarket pedals that go over or in place of the existing pedals. I drilled a couple of holes in my accelerator and attached a little tab on the side that allows me to easily reach it with my pinky.
If you have naturally splayed feet like I do, there's just no way to rotate your leg around enough to put your heel on the accelerator and still drive properly.
Get a pair of boots that have a wide sole and if if feels natural to blip the throttle with your pinky toe, than make one of the modifications I listed so you can do it with thinner shoes.

I think I'm getting it down. I'm more successful doing it on the nsx than the s2k. The pedals on the nsx are a bit closer and I have to use "ball of foot-heel" to make it work. On the s2k it's definitely "heel-toe" due to the wider spacing of the pedals. Pretty fun stuff!
