I spent some time looking at my pulley under a microscope today. I showed it to a few guys as well for input.
Its very hard for my to say why my pulley started to fail, but I "think" I know how the crack started. I want to say its starting to fail from what I think is the pulley bending or sort of folding back and forth. You can say wobbling, I guess.
This could have happened due to three reasons; again, in my mind:
1. Me not properly adjusting the bolts.
2. the bolts loosing torque and the pulley becoming loose.
3. overtightening when installing (me again) and again the bolts stretched and let go over time.
Even though I never make mistakes
, I am leaning towards #3. I used loctite blue on them ... so I probably overtightened them and caused the problem. They were not loose when I removed them.
I ordered 2 stock pulleys, because I think the stock pulley is more than enough once correctly installed and its stupidly cheap. That stamped steel pulley is hard, if you are going to change anything I would look more towards the bolts ... even though that might be opening a can of worms.
I do have a torque wrench that would have allowed me to properly torque it to 120 inch-lbs, but I failed to use it due to the fact that I did not have a way to hold the pulley the second time I had to remove my clutch.
I hardly doubt at this time that my pulley starting to fail is related to anything KW. And again theres is no one here that has a "virgin" KW kit and a pulley failure.

I cant ... but I am flying out to Michigan next week. I can bring it with me through the x-ray machine
